You must complete and submit this form to be exempted from the required $100 registration fee at registration.
The forms provided in the left column tell us what we need to know to keep your children safe and you parents and guardians informed of any concerns that might arise during school activities. Many of the forms are health related and provide the district with ways to get in touch with you in a hurry.
Consent forms need to be on hand when health workers arrive in the district to do procedures like vaccinations.
Completing the application for Free and Reduced Price School Meals allows the school district to pass on to your family the benefits of the National School Lunch Program (NSLP), a federal program provided by the USDA. We participate so that families get a break in food costs, and can put the money you save toward other essential goods and services for your family. Currently half of our students are participating. High participation qualifies our district for some significant discounts in other parts of the budget, so full participation of all eligible students is really helpful in these time of shrinking state aid.
Your student’s participation is confidential; there is no indication on anything in the food line process that gives any hint to any student who is participating in the program.
